Summary: Performs and assists with a variety of patient care duties and activities of daily living for residents in Terrace Grove Assisted Living under the direction of the head or charge nurse. 

Essential Duties and Responsibilities include the following.  Other duties may be assigned to meet business needs.

  • Assists with patient transfers, bathing, grooming, and other personal cares as needed
  • Assists nurse by taking vital signs, food and fluid intake/output as directed
  • Communicates effectively with residents and conveys respect and dignity at all times
  • Takes meal orders, serves meals, and assists with other dining needs
  • Passes mail to residents
  • Passes water to residents
  • Makes daily entries into communication log and charts a variety of patient information
  • Monitors call lights throughout facility and communicates effectively with other staff to ensure lights are answered in a timely manner
  • Assists with tidying residents' rooms as needed and as requested by the resident and/or family
  • Observes and reports any resident related issues such as medical, physical, emotional or social needs to the nurse or Administrator
  • Assures resident comfort
  • Conducts shift change report at the end of every shift to ensure a smooth shift transitions
  • Uses appropriate safety precautions, personal protective equipment, and adheres to universal precautions and infection control guidelines
  • Encourages residents to maintain the highest level of functioning
  • Monitors the facility and resident rooms to ensure a safe environment
  • Upholds patient and business confidentiality at all times.

Marginal Requirements:

  • May Provide backup phone support when needed

Job Requirements


  • Less than high school education required.



  • Requires valid Certified Nursing Assistant certification
  • Requires current CPR, First Aid (in-house training available)
  • Food Handler certification


Other Qualifications

  • Available to work rotating weekends and holidays
  • Requires excellent interpersonal communication skills
  • Ability to multi-task
  • Basic computer skills


Supervisory and Reporting Responsibilities

  • Reports directly to the Director of Nurses or nurse on duty
